REQUIRED CASE MATERIALS
For best results, please include the following with your case:
- Both maxillary and mandibular preoperative casts. Mark the outline of the required tissue contouring that will be performed at the time of prepping on the cast.
- Diagnostic wax-ups
- Bite Registration
- Full-arch impression trays preferred
- Smile design or request a smile catalog from us
COSMETIC CASE CHECKLIST
Checklist of Items Needed to Complete a Cosmetic Case:
- Photos of pre-op teeth with shade guide from different angles
- Photos of preps with stump shade tabs
- Photos of temporaries, full face including eyes.
- Photos of temporaries, eye to chin relaxed lip position
- Model of pre-op
- Model of temporaries
- Bite registration
- Full arch impression
- Stick bite (vertical and horizontal plane parallel to interpupillary line)
- Detailed lab slip expressing goals for your patient
- Indicate the desired length of final restorations
SHIPMENT PREPARATION
- Remove all previous markings and stickers on the package
- Disinfect all case materials, place them in the bag, and seal them
- Wrap each model individually with bubble wrap or foam padding
- Fill empty spaces with additional bubble wrap
- Remove models from the articulator, wrap them individually, anchor it in the box
- Face-bow and other enclosures should be wrapped separately
- Shipping any type of alginate: put Rx in a separate sealed plastic bag and staple to the bag
- Shipping loose crowns, bridges, implant parts, and shade tabs: Place in a separate small container or sealed plastic bag and staple to the Rx.
- Include the patient’s name and staple it to the Rx
- Place any photos or slides that accompany the case in a plastic bag.
- If shipping multiple cases in one box, bag each case separately
- Apply the shipping label to the case box and tape the box shut

INTRAORAL SCANNER PAIRING
The Dental Lab accepts scans from every type of intra-oral dental scanner. We can accept most .stl files for digital cases.

• Go to the section labeled “Find a Laboratory” on your iTero Scanner, or you can search the lab locator at www.itero.com
• Connect with us by using our Company ID 21051, and be sure to select our Lab when sending cases.

• We also accept cases from 3rd Party scanners such as; Midmark, BioBigBox, DDX, Heron Cloud and Shining 3D. You can find us on these IOS Portals by searching our Digital email at [email protected]. You may also send raw STL scans, DICOMS and zipped files directly to this email, providing you include Case Notes and pertinent information in your email.

Our products have a 5-year guarantee per the ADA regulations. The Dental Lab reserve the right to charge for the repairs or remake of any appliance returned to us that shows signs of alterations by others, misuse, and or abuse. We can not guarantee restorations, such as mouth guards, repairs, implant parts or any changes to the original prescription.
If the impression is not suitable, but is insisted upon being used, the restoration will not qualify for any guarantee. All dental appliances are custom made and can not be refunded. However, a credit will be issued and applied to your account for any case with problems pertaining to our laboratory work. The original case must be returned before a credit will be issued. In order for cases returned to be redone at no charge the cases must have the original appliance or restoration with the new impression. If a case is returned for a remake without the original appliance, we will charge your account. Once received, we will credit the amount onto your account balance.
If the impression is not suitable, but is insisted upon being used, the restoration will not qualify for any guarantee. All dental appliances are custom made and can not be refunded.
